In the beginning there is an establishing shot of the wilderness to show how vast it is and how large it is.

Juno is upset but i beleive she is morning over sarahs husband. The doctors and nurses do not seem to care that a patient is on the floor.

Reference to the shining as they go through the woods like in the beggining of the shining it is also to show how far they are away from civilisation.
